Your intake forms must be completed before the appointment takes place. You can access appointment forms at the top of David’s Calendar, or we can email you a PDF or Word doc. Refunds are not provided for appointments cancelled less than 24 hours before the appointment, including for the reason of missing intake forms.

If you’re looking for information on something specific, the subject line “Requesting Info on (Specific Topic)” is what catches our eyes first. Assuming you consent to email correspondence on the intake form, we send a follow up email after appointments with any instructions you might forget later (such as herbal suggestions and doses or links to meditation recordings), but if you need something clarified the best subject line is “Clarification on Appointment.”
When you email us to ask about scheduling an appointment, our desk master will ask about what your specific needs are unless you’re scheduling for the full holistic intake. The reason is we have different intake forms for different services. If you’re coming to work on trauma, for instance, we don’t really need to know much about your medical history. If you’re getting energy work, we don’t need to know what medications you’re on, but we will need to know if you’ve had surgeries (especially organ transplants). You see the reasoning. Amy answers all our intake emails and she’s excellent at assessing what your needs are. Alternatively, you can simply do a full holistic intake with the most inclusive intake form and we can reference that full information in the future should your needed services change.
